How do I access my Client Portal (billing, appointment requests, etc.)?

In your client portal you can see your appointment schedule, join video sessions, manage billing, fill out electronic paperwork and access shared documents.
How much do therapy sessions cost? Do you accept insurance?
Fees for a 50 minute session range from $110-$150 for individuals and $140-$180 for couples and families. Out of pocket fee is based a tiered fee system that considers your financial circumstances. Now accepting Blue Cross Blue Shield, United Healthcare, and Aetna. A Superbill can be provided for you to pursue out-of-network benefits.
What are the privacy practices for my health information?
Your privacy is of utmost importance and your health information will be held to the highest levels of confidentiality. Whole Congruence Therapy utilizes Simple Practice for electronic health record creation and storage, maintaining your private health information to the standards the law requires. Please contact Whole Congruence Therapy for the full Privacy Practices for Protected Health Information (PHI), and your agreement to these policies is required to start therapy.
What is required for gender congruence surgery evaluation and support letters?
Whole Congruence Therapy does not want to be a barrier to you accessing care. Ongoing therapy is recommended but not required to receive letters of support for gender congruence measures. A minimum of one session is required for the evaluation to gather sufficient information for your unique support letter and meet the WPATH standards of care and your surgeon's requirements. Insurance is not accepted for evaluation sessions. Additional fees may apply for the time required to complete letters.
What is Miriam's Education and Training?
Miriam completes ongoing continuing education to stay up to date on ethics, cultural competency, technology use practices, therapeutic modalities, and to continue challenging herself to grow as a person and therapist. She has specialized training in performing evaluations and writing letters to support gender congruence measures using the Gender Affirmative and Supportive Surgery Evaluation Tool (Gender ASSET) and she is an EMDR Trained Therapist to target a wide array of distressing issues including trauma. Her therapeutic experience covers a range of presenting challenges including identity exploration, depression and anxiety, eating disorders in families, premarital counseling, couples conflict, family and parenting struggles, trauma, and personal and life transitions.
Miriam achieved her master of science in Family Therapy from Texas Woman's University in 2018 and subsequently completed the licensing requirements to become a Licensed Marriage and Family Therapist (LMFT). Before finding and following her passion of helping people through life's many challenges as a therapist, Miriam completed a bachelor of science in Mechanical Engineering with a minor in Sexuality Studies from the University of California, Davis, and worked as a mechanical engineer for a few years. Her career path provides a unique perspective that is creative, analytical, normalizing, and often humor-filled.

How do I make a complaint?
Miriam Kolni is a Licensed Marriage and Family Therapist (Texas License #203258). The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.

Right to a good faith estimate:
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
